Credit Application Guidelines
To obtain a credit account with Toledo Spring Parts & Service, please download and complete the form below and remit to our office. The Credit Application Form can be submitted in person, by mail (5015 Enterprise Blvd., Toledo, Ohio 43612), via email (firstname.lastname@example.org), or by fax (419.729.7019).
We value your business and appreciate your request to open a credit account with us. Following these guidelines will ensure that the application is filled out correctly. Doing so will help to avoid delays in processing your application. Please be aware that incomplete applications cannot be processed and will be returned to the applicant for completion.

Please allow a few weeks for processing and note that sending in a credit application does not automatically open a credit account. Please click the form below and fill it out completely.
Statements – Statements are normally sent by fax or email on the 1st of the month. Please provide a fax number or email address in order to ensure that you receive your statements. If you do not have a fax or email , we will mail a copy of your statements to you at the address that your provide on your application.
Terms – Our terms for payment are net 10th. This means that anything that is purchased in the month is due the following month on the 10th (i.e. May Invoices are due on June 10th). We reserve the right to put an account on hold at anytime that we feel payment is falling behind.
